Business Grade ADSL Application Terms

  1. These services are supplied to you under a TWELVE (12) MONTH MINIMUM TERM CONTRACT, (longer terms available) by submitting this application you
  2. agree to be bound by the application terms of this contract and our Acceptable Use Policy (AUP).
  3. Once this application is submitted to us CHARGES WILL BE INCURRED, this application can not be cancelled without the payment of the termination fee (setup costs + 12mths at the nominated access plan rate).
  4. If you cancel this DSL service or wish to relocate the service to another premises or phone line within the 12 month contract period, You will incur the early contract termination fee ( balance of the 12mths (or contracted period) at the nominated access plan rate).
  5. Each ADSL link incurs a once off setup fee please refer to the online documentation for the relevant fees, (see business grade).
  6. This and all other West Australian Networks services must be paid for in advance, should your account fall into arrears it will be suspended until the payment is received. In the event of an overdue account West Australian Networks reserves the right to disconnect the account permanently, early disconnection and reconnection fees may be payable by you should this occur.
  7. Changing speed of link (Faster or slower) will incur a fee, please refer to the online documentation, (see business grade).
  8. Your ADSL link on average will be operational within 7 - 10 working days (depends on port availability).
  9. Account fees are not charged until the service is connected. Charges commence on the day of connection at the exchange not the date you commence using the service.
  10. Changes and cancellations must be advised in writing to West Australian Networks accounts prior to the end of any billing period (24th of the month) and will become effective as of the 1st of the following calendar month.
  11. ADSL accounts can not be suspended.
  12. You must accept the terms and conditions at the bottom of this agreement by ensuring you place a tick in the box. The application can not be processed with out this.

Terms are strictly payment in advance. (payments must be made prior to the 1st of each month).
Payment for the setup fee and first month must be supplied with this request form in order for us to accept and process it.
If you are paying by electronic funds transfer (EFT / pay anyone etc) the application will held until your payment is received.

If an application for service is not successful a full refund will be provided.

Fast Churn

If you have an existing ADSL service with an ISP participating in fast churn process they may take advantage of the faster setup time and lower connection fee.

Please Note: If a change of IP address for Layer 3B, 3C or 3D services, or a change of speed or service configuration is performed at the time of churn, the client may incur an additional charge.

NOTE: If you receive an error message reading: "The form was not submitted for the following reasons: You are coming from an unauthorized
domain. Please use your browser's back button to return to the form and try again".

This problem is caused by your personal firewall blocking HTTP referrer information (cookies) which the script checks as a security measure. When
that information is blocked by your firewall software, the form returns the error message you received.

To send the form, you can temporarily disable your firewall (which you should be able to do by right-clicking its icon in the system tray* and
